Sona Movement Studio – Participation Waiver
By booking or attending classes at Sona Movement Studio PTY LTD (Shop 2, 71 Ridge Street, Gordon 2072), I acknowledge and agree to the following:
Acknowledgment of Risks: I understand that Pilates (reformer and mat), yoga, Dance Fit, and Ballet Fit involve physical activity that carries some risk of injury. I confirm that I am physically capable of participating and have consulted a physician if necessary.
Waiver & Release of Liability: I release and hold harmless Sona Movement Studio PTY LTD, its owners, instructors, and staff from any liability, claims, or demands arising from any injury, loss, or damage sustained while participating in classes or activities.
Age Confirmation: I confirm that I am over 18 years of age.
Photo & Media Consent: I understand that Sona Movement Studio may take photos or videos during classes for promotional purposes. I agree to have my image used, or I can indicate otherwise by contacting the studio directly.
Ongoing Agreement: This waiver applies to all future classes and activities until further notice.
